This completed form and nominee/faculty curriculum vitae (CV) with updated IU appointment are required for all requests and will be submitted to Dr. Keith Avin, Associate Dean, IU Graduate School Indianapolis, for approval.

Request for Limited Status or Outside Member Approval Request for non-tenure track faculty or other appointee to receive Indiana University “limited graduate faculty status.” This can include adjunct, instructors and research track faculty who can be nominated to serve on limited or select student committees. Faculty has left or is leaving IU, therefore I am requesting “limited graduate faculty status” for this member to remain on a student’s research committee until the student graduates. Request for outside (non-IU) member approval. (Note: This would be considered a 5th member and may not be one of the minimum four and cannot serve as chair.)

A departmental advisory or steering committee must vote to nominate an Indiana University graduate faculty member for an endorsement to chair doctoral dissertation committees. Departments may also select another appropriate review process, such as a majority vote of the department’s graduate faculty members who hold the endorsement.

NOTE: Faculty who leave IU cannot remain on a committee as chair. They can move to co-chair or regular member. These situations require a Change of Research Committee Member Form.
