Graduate/Professional Program Update Form

This form is to be used to reactivate graduate or professional student status and update information in the university student system. Use this form if you have not had continuous enrollment since your admission.

NOTE: If you have been out 24 months or longer, you will need to reapply. Please go to the IU Graduate CAS Application to reapply.

If you are an undergraduate student looking to update your undergraduate/bachelor's degree status, please contact the registrar's office. This form does not serve undergraduates.

All information is required. Incomplete forms will be returned which will cause a delay in processing.
