IU Graduate CAS New Program Request Form

Please use this form to request a new graduate or professional application (designation) within the IU Graduate CAS for newly approved degrees or certificates. Only those new programs approved by the university / state, and thus have been added to SIS Academic Structure should be added to the IU Graduate CAS. You can obtain your program / plan / subplan code from your campus Registrar's Office or Graduate Office.

Once submitted a campus contact will reach out to you with instructions on next steps.
